REMINDER TO
ALL PARENTS
Those
students entering the 5th grade or the 9th grade need a physical that is
dated 8/26/07 or newer for the first day of school.
All
students in the 5th grade-------------------must have 3 Hepatitis B
vaccines
6th grade-------------------must have had a
varicella vaccine or have documentation of
having had
chicken pox disease
7th grade------------------ must have had 2
MMR's
8th grade------------------ must have a Td
that is less than 10 years old
Any student
wishing to play in fall sports in the 7th and 8th grade must have a
physical that is less than 23 months old.
